Câu chuyện trong game bắt đầu khi một thảm họa xảy ra và tiêu diệt gần như toàn bộ loài người. May mắn thay, Adam Collins không nằm trong số đó và anh quyết định quay trở lại thành phố Haventon để tìm kiếm người thân của mình. Với hi vọng nhỏ nhoi, Adam đã chiến đấu để giành giật sự sống và tương lai cho chính bản thân.
Gameplay của I Am Alive đi theo lối hành động giải đố, khá giống với dòng game Uncharted, cũng có nhưng pha lèo trèo và chiến đấu mạo hiểm, chỉ khác là rừng rậm nay đã được thay thế bằng những tòa nhà chọc trời và Adam không có nhiều thời gian cũng như sức lực bởi mỗi phút trôi qua, anh sẽ phải trả giá bằng lượng stamina ít ỏi của mình.
