Được phát triển bởi studio đã làm nên Mafia: The City Of Lost Heaven - Illusion Softwork (2K Czech), Mafia II sẽ đưa người chơi đến với Empire Bay, một thành phố giả tưởng của New York City những năm 1945-1955.
Empire Bay trong Mafia II sẽ được xây dựng như một thế giới mở với diện tích 10 dặm vuông (tương đương với 26 kilomet vuông) cho phép người chơi tự do di chuyển và khám phá mọi ngóc ngách trong trò chơi.Cùng với 50 loại phương tiện di chuyển khác nhau cùng với những bản nhạc phù hợp với thời kì mà trò chơi diễn ra.
Nhiều loại súng trong phần đầu sẽ được trở lại trong Mafia II, như khẩu Thompson M1928, súng Colt 1911 và shotgun. Thêm vào đó là những loại vũ khí của thời kì đệ nhị thế chiến như MP40 và súng tiểu liên M3, khẩu MG42...
Câu chuyện trong Mafia II kể về cuộc đời của Vito Scaletta. Sinh ra tại tại Siciy và sau đó cùng gia đình nhập cư vào nước Mỹ khi còn nhỏ. Cuộc sống nghèo khổ trên những con đường của một thành phố Mỹ rộng lớn đã làm cho Vito mong muốn được trở thành một nhân vật tầm cỡ, với bất cứ giá nào. Vito sau đó đã gia nhập vào một tổ chức tội phạm và nhanh chóng trở thành một thành viên tin cẩn.
Trong trò chơi sẽ có nhiều tình huống mà người chơi có thể lựa chọn nhiều cách hành xử, từ đó sẽ dẫn đến những kết quả khác nhau. Những lựa chọn của người chơi cũng sẽ dẫn đến những kết thúc khác nhau cho trò chơi. Những câu chuyện trong Mafia II không chỉ là những phi vụ làm ăn, những pha rượt đuổi trên đường phố, mà còn là sự cạnh tranh làm ăn của 3 tổ chức tội phạm khét tiếng: Falconi, Vinci và Clemente.
Daniel Vávra, người viết kịch bản và đạo diễn cho Mafia II đã nói rằng:” phiên bản trước của trò chơi là một sự bày tỏ lòng kính trọng tới những bộ phim về gangster, với cái nhìn lãng mạn. Mafia II sẽ thực tế hơn, một thế giới đen tối, và những hiệu ứng của nó được xây dựng dựa trên thực tế”.
Với những gì nhà phát triển hứa hẹn, Mafia II sẽ là tựa game rất đáng trông đợi trong năm 2010.
